CTG understands the unique challenges that the COVID-19 vaccination program may present to your pharmacy. As these vaccination efforts progress, we want to highlight existing resources and insights that can currently assist you in your pharmacy in preparation and participation in Operation Warp Speed:

• Pharmacies administering the COVID vaccine will be required to report vaccine information to their state Immunization Reporting Systems. State Reporting System information can be found here.

• Pharmacies will also be required to obtain a signed Resident Consent Form either by resident or POA/Legal Guardian.

• Key Reporting Items include: Patient Name, Patient DOB, Patient Race, Patient Ethnicity, Facility Name, Facility Address, Facility Phone Number, POA/Legal Guardian Name, POA/Legal Guardian Contact Information, Patient Insurance Carrier, Insurance ID, Insurance Group, SS#, Gender, PCP Name, PCP Phone Number, PCP Fax Number, Administration Site, Dose Number, Drug NDC, Lot Number, and Expiration date, Pharmacist or Technician administering vaccine along with license number.

• Facility administrators should work with staff to report any health problems that occur after vaccination to the Vaccine Adverse Event Reporting System (VAERS).

• As an alternative, LTCF Staff and Residents with smartphones are encouraged to enroll in V-safe. V-Safe is the application that the CDC has developed for individuals that receive the vaccine to report any adverse side effects. V-safe is a new smartphone-based, after-vaccination health checker for people who receive COVID-19 vaccines. V-safe uses text messaging and web surveys to allow vaccine recipients to tell CDC how they feel after getting a COVID-19 vaccine.

• If your pharmacy will not be conducting vaccinations in the initial phase one request that our LTC pharmacies have faced is to provide residents insurance information including Insurance Name, Insurance ID, Group number, BIN, and PCN.
